Job Responsibilities

Under general supervision, provide direct nursing care to clients exclusively enrolled in the Ryan White program which includes:   screening patients for eligibility, medical and non-medical case management, including treatment adherence, evaluation, counseling and providing a broad range of services that link clients to sub-specialty services.  Responsible for case management intake assessments and performing comprehensive assessment of health care needs.
 
Duties:

• Fulfill nurse protocol training requirements in HIV, STD, Immunization, and other relevant areas to function as an expanded role PHN.
• Provide direct nursing care including physical assessments, education, treatment, and develop and document a plan of care consistent with the clients’ level of need.  
• Screen and refer clients to other community service providers as needed.  Assist with referrals and follow-up treatment planning as indicated.  
• Provide sexual health promotion and prevention education, including counseling clients and family regarding the importance of medication adherence for viral suppression.  
• Participate in staff meetings and assist with developing and implementing quality improvement projects as needed.
• Maintain accurate medical records, documenting is a must, assist with data entry and quality assurance monitoring.

Minimum Qualifications

Associate degree in nursing from an accredited college or university AND one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification;

OR

One (1) year of experience at the lower level PH Registered Nurse 1 (HCP190) or position equivalent.
